Documents for registering

Students can scan in documents and attach to the online DSS registration form, or email them to dss@cit.ie or you can give a photocopy of your documents with evidence/diagnosis of your learning difference, health condition or disability into the Access Service (Student Centre) where the DSS is based.

  • If you have already submitted your documents to the CAO you do not need to bring in another copy unless you have new or updated information.
  • Please submit as much information as possible including full diagnosis for registering with the DSS.
  • Funding can be requested from the Fund for Students with Disabilities (FSD) by the Disability Support Officer on behalf of eligible students who have provided evidence of disability documentation that meets the criteria for the Fund - see this webpage for more information: www.mycit.ie/access_disability/disability-support-service/financial-support

Documentation required:

When submitting your evidence of disability documentation please make sure that it has been completed by the appropriate medical professional for your disability.  A list of the appropriate professionals for each disability type (e.g. blind/vision impaired or dyslexia) is provided in the table below.

Students who are unable to provide the disability evidence from the source specified in the table below can avail of a general level of disability support (e.g. Exam Accommodations, Academic supports and advice on assistive technology and access to a disability advisor) by providing evidence of a disability from a General Practitioner or other health professional (e.g. Psychologist). Students with a Specific Learning Difficulty, who do not have a full report from an Educational Psychologist, may present evidence of a history of a specific learning difficulty or receiving educational supports e.g. as part of the DARE Educational Impact Statement or State Examinations Commission letter regarding RACE (Reasonable Accommodations at the Certificate Examinations) accommodations.

FSD eligibility- Where the above person has provided a diagnosis of autism or Asperger’s syndrome. 

Age of report: No age limit.

Attention Deficit Disorder/Attention Deficit Hyperactivity Disorder (ADD/ADHD)

Report to be provided & signed by:

  • Appropriately qualified Consultant Psychiatrist or Psychologist or Neurologist or Paediatrician who is a member of their professional or regulatory body.

FSD eligibility- Where the above person has provided a diagnosis of ADD/ADHD.

Age of report: No age limit.

Or completed Evidence of Disability form

Blind/Vision Impaired

Report including diagnosis from an Ophthalmologist or Ophthalmic Surgeon.

FSD eligibility- One or more of the following:

  • Evidence of disability from an Ophthalmologist/Ophthalmic Surgeon, with signature;
  • Letter from the National Council for the Blind confirming registration with the Council;
  • If a student has attended a school for the Blind, a letter on headed notepaper signed by the principal which confirms attendance at the school.

The report should confirm The Best Corrected Visual Acuity is equal to or less than 6/24 (Snellen) in one eye
OR
Near Vision N18 or less in one eye.
OR
The Peripheral Field of Vision is limited to the extent that it interferes with normal visual acquisition of visual material e.g. Homonymous Hemianopia.
OR
The Central Field of Vision is limited to the extent that it interferes with normal visual acquisition of visual material e.g. Stargardt’s Disease
OR
Cortical visual impairment as part of a brain insult resulting in an inability to process visual information

Deaf/Hard of Hearing

Students may apply

under one of the

following categories:

(A) Applicants who

have an Audiogram

(B) Applicants who

attend a School for the

Deaf

(C) Applicants with a

Cochlear Implant

(A) Applicants who have an audiogram: Diagnostic/ Clinical Audiologist registered with the Irish Academy of Audiologists (IAA) or HSE Audiologist

(B) Applicants who attend a School for the Deaf: Principal of School for the Deaf

(C) Applicants with a Cochlear Implant: Ear, Nose and Throat (ENT) Consultant OR Cochlear Implant Programme Co-ordinator

Evidence from high street retailers is not acceptable.

FSD eligibility- One or more of the following:

  • An audiogram from a professionally qualified Audiologist and/or ENT Consultant, with signature, clearly indicating moderate to profound bilateral hearing loss (i.e. above 40dB).
  • If a student has attended a school for the Deaf, a letter on headed notepaper signed by the principal which confirms attendance at the school.

Age of evidence of disability: No age limit.

Or completed Evidence of Disability form

Mental Health Condition 

(including, but not exclusive to, the following: Bipolar Disorder, Schizophrenia, Clinical Depressive Conditions, Severe Anxiety, Severe Phobias, OCD, Severe Eating Disorders and Psychosis)

Report to be provided & signed by:

  • Consultant Psychiatrist or Specialist Registrar who is a member of their respective professional or regulatory body.

FSD eligibility- The report from the above should diagnose a significant mental health condition.

Age of report : The report should be no older than five years from the date of needs assessment.

Or completed Evidence of Disability form

Neurological Condition 

(including, but not exclusive to, the following: Epilepsy, Guillain-Barre Syndrome, Huntington’s Disease, Brain tumour, Multiple Sclerosis, Motor Neuron Disease, Friedrich’s Ataxia, Head/Traumatic Brain Injury, Stroke, Parkinson’s Disease, Spinal Muscular Atrophy)

Report to be provided & signed by:

  • Neurologist or other relevant Consultant.

FSD eligibility- The report from the above should diagnose a significant neurological condition.


Age of report: No age limit.

Or completed Evidence of Disability form

Speech, Language and Communication Disorder (Neurological Condition)

Report to be provided & signed by:

  • Speech and Language Therapist. 

FSD eligibility- The report from the above should diagnose a significant neurological condition.


Age of report: No age limit.

Or completed Evidence of Disability form

Significant Ongoing Illness 

(including, but not exclusive to, the following: Diabetes Type 1, Cystic Fibrosis, Severe Crohn’s Disease, Severe Ulcerative Colitis and IBS, Chronic Fatigue, Cancers)

Report to be provided & signed by:

  • Diabetes Type 1: Endocrinologist or paediatrician
  • Cystic Fibrosis: Consultant respiratory physician or pediatrician
  • Gastroenterology Conditions: Gastroenterologist
  • Others: Relevant Consultant in area of condition, or Consultant Registrar.

FSD eligibility- The report from the above, as appropriate, should diagnose a significant ongoing illness.


Age of report: The report should be no older than five years from the date of needs assessment.

Or completed Evidence of Disability form

Physical/mobility

Report to be provided & signed by Orthopaedic Consultant or other relevant specialist appropriate to the disability/condition (e.g. Pediatrician or Rheumatologist).

 

FSD eligibility: The report from the above should diagnose a significant physical or mobility difficulty. 


Age of report:  No age limit.

Or completed Evidence of Disability form

Developmental Co-ordination Disorder DCD (Dyspraxia/Dysgraphia)

Full psycho- educational assessment by appropriately qualified Psychologist

AND

Report by appropriately qualified Occupational Therapist/Neurologist/ Chartered Physiotherapist

 

FSD eligibility requires both of the following: 

  1. A full psycho-educational assessment from an Appropriately qualified Psychologist who is a member of their respective professional or regulatory body diagnosing Developmental Co-ordination Disorder, and
  2. A report by an apropriately qualified Occupational Therapist/Neurologist who is a member of their respective professional or regulatory body diagnosing Developmental Co-ordination Disorder.

All tests used in the assessments must be current, valid, and reliable and age appropriate.  The CAO/DARE Evidence of Disability form is not acceptable evidence of disability for FSD funding.

Specific Learning Difficulties (Dyslexia or Dyscalculia)

Full educational psychology assessment report to be provided & signed by:

  • Appropriately qualified Psychologist who is a member of their respective professional or regulatory body; or, an assessor who is accredited by PATOSS.

FSD eligibility - Full psycho-educational assessment from the above diagnosing a Specific Learning Difficulty. All tests used in the assessment must be current, valid, and reliable and age appropriate. 


Age of report: The report should be no older than five years from the date of needs assessment.

NOTE: If the tests referred to in the report have clearly been conducted using Adult Scales, then the five year time limit will not apply.

In the case of older reports additional evidence of relevant attainment scores or a communication from the State Examinations Commission regarding RACE will be required. Such information is available within the DARE Educational Impact Statement completed by schools.
